South Western Railways
Southern Railways
Konkan Railways
Western Railways comes in a close fourth.

These organisations are run by professional railwaymen and women.

And where, then, might you find the kind of clowns who ignored multiple checks and processes, and caused the Balasore catastrophe?

South Eastern Railway and Eastern Railway top the list, of course
Northeast Frontier Railway
And the great East Coast Railways, under whose aegis Balasore falls
North Eastern Railways brings in the rear

What does this mean? It means that I'd hop into an SWR or SR train without a second thought. They'd go above and beyond to ensure my safety.
